I acknowledge that I have been diagnosed with and I have chosen to explore Ayurvedic treatment as an alternative or complementary approach to my conventional medical care.

I understand that Dr. Praveena's Ayurvedic treatment plan involve a combination of herbal remedies, dietary modifications, lifestyle interventions. and other Ayurvedic modalities. I am aware that these treatments may be different from the conventional medical treatments I have received or may receive in the future.

I understand that the Ayurvedic approach aims to address the root causes of my condition and support the body's natural healing processes However, I acknowledge that the outcomes of Ayurvedic treatment may vary, and there are no guarantees of a specific result or timeline for improvement.

I have had the opportunity to discuss my concerns and ask questions about the Ayurvedic treatment process, and I am satisfied with the information provided by Dr. Praveena. I understand that I and Dr. Praveena have the right to refuse or discontinue the Ayurvedic treatment at any time.

I agree to follow the treatment plan and recommendations provided by Dr. Praveena to the best of my ability, and I will promptly inform her of any changes in my condition or concerns that arise during the course of treatment.

I hereby release Dr. Praveena and her associated healthcare providers from any liability related to the Ayurvedic treatment.
